Charitable Solicitation Complaint Form

If you would like to file a complaint about a charitable organization, professional fundraiser or nonprofit raffle, or about any solicitation that you have received for a charitable contribution, please submit your Charitable Solicitation Complaint online.

If you would prefer to complete the form and mail it in with your supporting documentation, please complete the downloadable Charitable Solicitation Complaint Form (PDF) and mail it to:
